JOB RESPONSIBILITIES

Many patients are unable to travel to healthcare facilities and need you, as a nurse, to come to their homes instead. This is to provide care based on the individual's needs. We are looking for inspired, engaged, and skilled nurses who want to work in home healthcare. As a nurse in home healthcare, you enable patients to receive medical care and nursing at home, allowing them to stay at home in a safe and dignified manner. We work in teams with doctors, physiotherapists, occupational therapists, home care providers, and other caregivers. Additionally, you will have close communication with the patient and their relatives. All with the aim of providing good and close care. As a nurse in home healthcare, you will have a central and important role for our enrolled patients.

Your job responsibilities include:

  • Performing health and medical interventions for patients in home healthcare
  • Being responsible for nursing and conducting medical assessments
  • Working in a team around the patient with, among others, physiotherapists, occupational therapists, and doctors
  • Planning and conducting home visits and follow-ups
  • Guiding and delegating nursing tasks to nursing staff and students
  • Documenting in Treserva and registering information in other operational systems
  • Working preventively to promote the patient's health and well-being
  • Working in our consulting operations during evenings and weekends

The position is primarily daytime work, but the schedule also includes evening and weekend shifts. You will then work in our consulting operations, which includes the municipality's care and nursing homes, home healthcare, as well as service and group homes within LSS and social psychiatry.

QUALIFICATIONS

We are looking for you who are a licensed nurse and have worked as a nurse for at least two years. You should have good knowledge of the Swedish language in speech, writing, and reading comprehension, as the job requires documentation and other administrative tasks. A driver's license for a manual transmission vehicle and driving experience is required as you will be driving to patients' homes.

It is an advantage if you have experience in municipal health and medical care and if you are familiar with the systems Treserva, TES, Lifecare, NPÖ, and Pascal.

To succeed in the role of nurse, you are flexible and can quickly adapt and make decisions based on the patient and their needs. You work safely with patients, take initiative, and are responsible for ensuring that your tasks are completed. You are a structured person who sees what needs to be prioritized, is efficient, and uses resources to achieve the best results. Furthermore, you have good communication skills and adapt your language to the recipient. You are confident in your role as a nurse.

As we work in teams in close collaboration with patients and relatives, you are skilled at creating and maintaining good relationships, both internally and externally. With good teamwork skills, you handle work situations with sound judgment and high integrity. You inspire trust, create confidence, and spread calm. You see opportunities in changes.
